Are you ready to schedule PT?

PLEASE READ

-Time......We are busy.  We have kids, jobs, homes, projects, and parents who may need us.  Please ask yourself if you have time to complete pelvic therapy before scheduling. I usually complete an evaluation and then weekly visits for a period of at least 4 weeks for 1-hour sessions.  Please see my hours to make sure there is a time slot that will work for you.  If you are scheduling on the website, please request an evaluation and 4-6 visits weekly following the evaluation.  

-Resources.....Prior to scheduling, please make sure that you know whether your insurance will cover PT and if so, do you have to meet your deductible before they will help with the costs or not?  This can be found by simply calling your insurance carrier.  I can tell you that Medicaid plans, as well as commercial insurance-based Medicaid plans, normally cover all of the costs of PT.  Medicare of KY normally covers 80% of PT.  However, if you have a Medicare Advantage Plan or commercial insurance, your coverage may vary unless you have met your out-of-pocket maximum.  I DO NOT BILL SECONDARY INSURANCE.  Please see my pricing section on this page for more information about what questions you may want to ask your insurance.  I do not take Passport or UHC Community Plan, Tricare, UMR, VA, or Medben insurance.

-Commitment.....You will be asked to do specific exercises which may take 15-30 minutes 6 days per week for the first 4-6 weeks.  You may be asked to keep a bowel or bladder diary.  You must be willing to take the time to fully complete your intake paperwork including all demographic, insurance, and contact information prior to the evaluation.  

-Cancellations.....You will be required to put in a credit card when you schedule as a new patient and you will be charged a $40 late cancellation fee to that card if you fail to cancel less than 24 hours before your appointment or do not show up for the appointment.  You will have an opportunity in your intake paperwork to sign up for a 24-hour appointment reminder.  I never schedule more than 6 patients per day in a 1-hour block, one on one with me, and I take care of all scheduling and billing.  This is a wonderful business plan for knowing what is going on with your business and providing quality health care but a terrible business plan when people cancel on the day of their appointment.  Most days, I am too busy seeing patients to try to fill an empty spot on the schedule when appointments are canceled on the day of the appointment.  I have found that many people find this fee to feel punitive but it is actually just a way to keep the doors open.  You will be automatically charged the late cancellation fee regardless of the reason for cancellation and you must be willing to sign a sheet in your intake paperwork as well as authorize your card to be charged in this case.  I will not charge your card otherwise without discussing it with you first.  If you do not feel comfortable putting in your credit card, you may put a "cancellation" deposit down when you schedule by either bringing a check by the office or mailing it to me to hold your appointment.  If you complete your plan of care without a cancellation that is within the 24-hour window or a no-show, then I will return the check or cash to you at discharge.  Medicaid does not allow me to charge their members a late fee so I allow one grace "late cancellation or no-show" but automatically discharge the patient if a second one occurs.
